If you need Skilled Nursing you are eligible for the following services:
· Skilled nursing services
· Physical therapy
· Occupational therapy
· Speech therapy
· Hospice care
· Drug administered by professional and licensed staff
· Medical and surgical supplies ordinarily provided
· X-rays and other radiology services ordinarily provided
· Use of appliances such as wheel chairs and positioning equipment
· Physician and Practitioner services
· Meals, including special diets
· Semiprivate room or private room if necessary

Rehabilitation

The strong touch and healing nature of our therapy staff will work to:
• Restore strength after post-operative procedures
• Help residents find success through a holistic approach to care
• Provide transitional support showing understanding and compassion
• Invite families to participate in therapy to provide the best care possible
• Engage residents to strive for improved health and work within the capabilities of each individual
• Bring honor and dignity as part of our clinical team
• Home evaluations prior to discharge

We offer rehabilitation services up to 7 days a week.

Personal Care

A premise in which food, shelter, and personal assistance or supervision are provided for a period exceeding 24 hours, for four or more adults who are not relatives of the operator, who do not require the services in or of a licensed long-term care facility, but who do not require extensive assistance or supervision in activities of daily living or instrumental activities of daily living.

At Broad Acres Health and Rehabilitation and Country Terrace Personal Care Home, programming and treatment plans are individually developed to assist individuals who need physical, occupational, or speech therapy, but do not need to be in an inpatient setting.
